Individual/Group Waiver & Release of Liability Agreement

At Dynamic Xplosion Cheer, Inc. you can choose to pay for classes monthly or weekly. Weekly payments are due one week in advance. Monthly payments must be paid monthly by the due date. There will be no accepting payments on the day of classes. There is no additional charge for months that have more than the normal number of class days. There is also no deduction for months that have less than normal class days, or holiday closings. 

 

Dynamic Xplosion Cheer Inc. (“Dynamic Xplosion Cheer”) is not responsible for any injury (or loss of property) to any person while practicing, training, taking class, competing, participating in open gym, special events, demonstrations or shows, or in any other way involved in cheerleading, tumbling, or teams (the “activity”) at Dynamic Xplosion Cheer for any reason whatsoever. Being fully aware of these dangers, I hereby give consent for my child(ren) or team to participate in any and all of Dynamic Xplosion Cheer Inc., activities and I ACCEPT ALL RISKS associated with any activities or participation.

In consideration for my or my child(ren)’s participation, I hereby, for myself and my child(ren) and our respective heirs and successors PROMISE NOT TO SUE and FOREVER RELEASE Dynamic Xplosion Cheer Inc., its officers, directors, coaches, and volunteers from all liability resulting in damages or injuries incurred as a result of participation including those resulting from acts of negligence.

I am aware that individual and group publicity photos and videos are taken from time to time and in considerations for my child(ren)’s or team participation I hereby grant my permission for my child’s or team likeness to be used in Dynamic Xplosion Cheer Inc. publicity or advertising.

In the event of an accident or emergency I herby authorize my child(ren) or athlete(s)to be transported to a hospital for medical treatment and I hold Dynamic Xplosion Cheer Inc. and its representatives harmless in the execution of such. Additionally, I hereby agree to individually provide for all medical expenses which may be incurred by myself, my child(ren), or athlete as a result of injury sustained while participating at or for Dynamic Xplosion Cheer Inc.

 

I have read and understand this ASSUMPTION OF RISK and WAIVER OF LIABILITY and PHOTO RELEASE and I VOLUNTARILY affix my name in agreement.
